Concorde Medical Group, NHPP welcomes you to our HIPAA SECURE patient portal - a password protected web communication tool. With this portal we provide you with the best tools available to securely manage your health records and communicate with your provider.


PATIENT PORTAL ACCESS
Our secure, HIPAA-compliant patient portal is automatically activated for any Concorde patient with an appointment scheduled and an email address on file. If you have not provided our office with an email address and wish to have access to your portal account, please contact your provider's office for assistance. At this time, proxy access is not available for children between the ages of 13 and 18 years old. To request healthcare proxy access, please complete the proxy forms for an adult or minor (12 years old and under), then email the completed forms with the requested documentation to [email protected] or fax them to 212-253-9631. Once you send the request, you must call your provider's office during normal business hours to alert them to the request. 

Please note: [email protected] is only used to accept ezAccess proxy requests and the Authorizations for the Release of medical records. Please contact your provider directly by messaging through the ezAccess portal once you've created your account or call the office directly for any other requests. Please call ezAccess Portal Support if you need further assistance setting up your ezAccess account. 

Passwords expire every 180 Days. Please log in on both the Desktop and Mobile App with your new password once you've reset it. At times the mobile app can take longer to sync with the pasword update if you have opted in to mobile app access without password entry each time you log in.

You may find that you will not be able to access every report under the "Medical" or "Documents" sections. There are times that those reports will need to be retrieved from a separate application that is not directly connected to our electronic medical records system. Please send a message to your Provider and we will respond with those reports attached to you as a PDF.

NEW PATIENTS
New patients will be emailed a link to join the patient portal after their first appointment.

Medical Records Request
If you would like to request your full medical records from Concorde, please complete the Authorization for Release of Health Information form.


*The patient portal is not intended for urgent requests. For urgent requests, please call your provider's office. 
If you are experiencing a true medical emergency, please call 911 or go to the nearest emergency room.
