Important ezAccess patient portal updates

  • As of Saturday, November 1, 2025 the ezAccess web version has transitioned to view-only mode. This means you can still see your prior records and message history online. Please note this information will not be moved over to MyNorthwell.The ezAccess mobile app is no longer active.
  • As of Friday, October 31, 2025 the ezAccess mobile app is no longer accessible as we transition to MyNorthwell. Please use the ezAccess website version to review chart updates and provider messages.

Concorde Medical Group, NHPP welcomes you to our HIPAA SECURE Read-Only patient portal - a password protected web communication tool. With this portal we provide you with secure access to your historical ezAccess Health Records.

Medical Records Request
If you would like to request your full medical records from Concorde, please complete the Authorization for Release of Health Information form.


To request healthcare proxy records, please complete the proxy forms for an adult or minor (12 years old and under), then email the completed forms with the requested documentation to [email protected] or fax them to 212-253-9631. If you are the patient’s legal guardian or Durable Power of Attorney, please attach to this form copies of any legal documentation demonstrating that you are the guardian or Power of Attorney, if such documentation is not already on file in the patient’s medical record. Once you send the request, you must call your provider's office during normal business hours to alert them to the request. 

Please note: [email protected] is only used to accept ezAccess proxy requests and the Authorizations for the Release of medical records.
 

Passwords expire every 180 Days. Please log in to the ezAccess website with your new password once you've reset it.
