Important updates about ezAccess
- As of Saturday, November 1, the ezAccess Patient Portal has fully transitioned to Read-Only. However, you will be able to login to the ezAccess website to view prior records and message history. Please note, this information will not carry over to MyNorthwell.
 - Starting Friday, October 31, the ezAccess mobile app will no longer be accessible as we transition to MyNorthwell. Please use the ezAccess website version to review chart updates and provider messages.
 - As of 10/30/2025, new ezAccess accounts will not be created as we are transitioning to a new portal, MyNorthwell. To learn more about MyNorthwell and to create your MyNorthwell account, click the link above.
 - Starting Tuesday, October 28, the ability to send or reply to messages with your providers in ezAccess will temporarily be paused until Northwell’s new digital system fully launches in early November. Until then, you will continue to receive chart updates and provider messages and can call your doctor's office directly with any questions.
 - Once MyNorthwell is fully launched, you will have full access to communicate with your provider(s) along with additional features, such as managing visits, accessing test results and more.

Welcome to Our ezAccess Patient Portal 
 
Concorde Medical Group, NHPP welcomes you to our HIPAA SECURE patient portal - a password protected web communication tool. With this portal we provide you with the best tools available to securely manage your health records and communicate with your provider.
 
 
 
PATIENT PORTAL ACCESS
At this time, proxy access is not available for children between the ages of 13 and 18 years old. To request healthcare proxy records, please complete the proxy forms for an adult or minor (12 years old and under), then email the completed forms with the requested  documentation to [email protected] or fax them to 212-253-9631. Once you send the request, you must call your provider's office during normal business hours to alert them to the request. 
Please note: [email protected] is only used to accept ezAccess proxy requests and the Authorizations for the Release of medical records. Please call ezAccess Portal Support if you need further assistance with setting up your ezAccess account. 
 
 
Passwords expire every 180 Days. Please log in to the ezAccess website with your new password once you've reset it. 
Medical Records Request
If you would like to request your full medical records from Concorde, please complete the Authorization for Release of Health Information form. 
*The patient portal is not intended for urgent  requests. For urgent requests, please call your provider's office. If you are experiencing a true medical emergency, please call 911 or go to the nearest emergency room.
